Have you earned your 2022 Wellness Benefit?

1. Log into sanfordhealthplan.com/memberlogin

• Forgot username and password options are available, if necessary. If you do not have an account, select the “Request Access for Yourself” button. 

2. From the top menu icon, scroll down to Insurance, click Portals and Links

• Earn points in the Wellness Portal by December 28
• Redeem points or check benefit balance in the Redemption Center by December 31
• Benefit balances may not apply if you are receiving fitness reimbursements
