NDPERS can only provide confidential information subject to NDCC 54-52-26 and 54-52.1.
A member may authorize the release of confidential information to another individual(s) by submitting the Authorization to Release Confidential Information (SFN 61324) or by providing written consent to NDPERS.
The written consent must include:
- member name,
- PERS ID number or Social Security number,
- specific plan(s) and information to be released,
- name and contact information of the person authorized.
The written consent must be signed and dated by the member.
